Case Management Services

Mental Health Targeted Case Management, or MH-TCM, is a specialized service aimed at helping adults with serious mental illness (SMI) and children with severe emotional disturbances (SED) access a variety of essential services. These services cover medical, social, educational, vocational, and other critical areas that directly impact an individual's mental health.

We are dedicated to collaborating with individuals and families, recognizing that each has unique needs. Our goal is to provide customized support that includes assessment, care coordination, educational resources, advocacy, and connections to vital community services.

We strive to empower families and individuals by equipping them with the necessary tools for long-term success. This support involves navigating the complexities of mental health challenges while promoting resilience and self-sufficiency.

Our team of committed mental health targeted case managers employs a person-centered approach, working diligently to assess needs, create effective plans, make referrals, coordinate care, and help individuals access a wide range of services. These services encompass mental health support, medical care, social services, educational opportunities, recreational services housing assistance, and vocational training.

By prioritizing individual needs and utilizing a network of resources, we aim to enhance the quality of life for those we serve, ensuring they have the support required to lead fulfilling lives within  their communities.

Program Eligibility & Services

  • All ages

  • Live in the Twin Cities metro area.

  • Adults  and children must have mental health diagnosis

  • must request case management services

  • Individual must have PMAP insurance

Here are some ways a case manager can help:

  • Determine the client's priorities.

  • Assess strengths and needs.

  • Promote housing stability.

  • Facilitate access to psychiatric and therapeutic services.

  • Collaborate with other providers to meet health and social requirements.

  • Oversee and coordinate services.

  • Develop crisis intervention plans.

  • Assist with Social Security and financial aid programs.

  • Help clients navigate everyday life challenges.

  • Pursue resources for work, volunteer, and/or school opportunities.

What's considered an SED Process

  • Assessment to determine client needs

  • Planning to create an individual community support plan (ICSP) for an adult or an individual family community support plan (IFCSP) for a child

  • Referrals and connections made with other services

  • Coordinating and monitoring services
